X-Ray Consultants, Inc., is an approved provider for most major insurance carriers and health plans, including Medicare and Medicaid. We are in-network for all contracts held by Select Health Network. We will submit both primary and secondary insurance claims to insurance companies on our patients’ behalf.

To streamline the registration process and to minimize your personal expenses, it is important to bring the following information with you at the time of your appointment:
- Your insurance carrier’s policy regarding pre-authorizations or referrals.
- Your complete insurance information, including:
- Insurance card
- Name and address of the plan or insurer(s)
- Name and address of the claims payer (if different from insurer)
- ID and group numbers
- Name of the policyholder or subscriber
